IMMEDIATE OPENING ….. 2- Community Health Worker (CHW), full-time and part-time

MYCAP is seeking to hire 2 qualified Certified Community Health Worker 1- full-time 40 hrs, and 1 part-time 30 hours per week, Monday-Thursday, $15.25 an hour to perform activities and services under the Mahoning Valley Pathways HUB utilizing the HUB Model and Pathways Stragegy; enrolling those with chronic conditions such as diabetes or prediabetes, hypertension, cardiovascular disease. Must be able to advocate for clients to obtain services, critical thinking skills to analyze and solve problems; ability to do home visits as needed requires public speaking and attendance at workshops, training and meetings as required; must be computer literate and have the ability to learn new software, tracking systems. CHW Certification a plus, or must seek certification within the time required. Must have reliable transportation and an insurable driver’s license, EEOE, DFW.
