Primary Stroke Center Coordinator – Part Time

Core Competencies for the Position

Makes referrals to other areas as appropriate, i.e. Quality Improvement, Risk Management, Infection Control

  • Assists in the development of clinical pathways, care planning process, standardized order sets for use by physicians and nursing staff
  • Develops internal benchmarks for comparison to external benchmarks
  • Communicates and Documents pertinent findings/activities in the patient's medical record
  • Collects and reports data accurately as required for Primary Stroke Center (PSC) Certification
  • Collaborates with Medical Staff and Hospital Staff to optimize outcomes for stroke patient
  • Stays abreast on emerging research and best practices in stroke care
  • Educates staff regarding best practices in stroke care
  • Identifies areas of improvement in stroke care and initiates measures to optimize performance and outcomes
  • Collaborates with Enterprise Stroke Program optimize service and outcomes
  • Assists in the education of the community regarding early identification and intervention for stroke
  • Completes continuing education as required of a PSC Team Member as specified in PSC Policy “Union Hospital Stroke Team Plan”

POSITION QUALIFICATIONS:

  1. Bachelor’s degree required.
  2. Licensed to practice as a Registered Nurse in the State of Ohio
  3. Experience in stoke care program management preferred
  4. Five years of nursing or other clinical healthcare experience
  5. BLS-C
  6. Experience working with Joint Commission, DNVGL, HFAP, CMS surveys with a leadership/coordination role preferred
  7. Experience in the care of the acute stoke patient
  8. Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision
  9. Able to work in a stressful environment and take decisive action
