Heart and Home Health Care is currently seeking qualified and compassionate nursing staff. The ideal candidate will have Home Care experience, excellent interpersonal skills, and the ability to work well with the interdisciplinary team. Duties and Responsibilities:

• Assess and Chart observations of patient’s condition at each visit
• Complete evaluation tasks, including medication reconciliation and vitals.
• Administer Practioner-prescribed medication
• Dress wounds and assess the healing process
• Provide patient and family education
• Coordinate with Practitioners, Physical Therapists, Occupational Therapists and others for Care Plan development
• Perform Supervision of Staff

Requirements:

• Current and unencumbered Ohio RN license
• Valid Driver License
• Current CPR certification
• Reliable transportation
• Pass Criminal Background Check and Urine Drug Screen
