Senior Benefits Case Manager
12 month, Senior Levy Grant

MYCAP is seeking an experienced and energetic Senior Case Manager that will perform professional advocacy/case management work with senior citizens and / or families in Mahoning County. A minimum of 2 yrs. experience working in a non-profit social service organization providing direct services to customers, an Associate degree in social work or applicable field; bilingual/licensed social worker or related field certifications a plus.

The Case Manager is required to ensure the seniors have access to the community resources needed , such as assisting with applications and processes to obtain or improve financial and health benefits; perform wellness calls or follow-up calls to seniors; make calls on their behalf to the resource agencies in the community to obtain information they need or to assist them to resolve a situation, attend recreation activities, plan senior informational events and individual needs assessments. Home visits are required for the disabled seniors needing services. Must be computer savvy, excellent reading, communication and writing skills; requires the ability to learn specific program software and data entry skills to report program outcomes, detail and goal oriented to meet grant/program compliance. Excellent people skills required with compassion, empathy, and effectively works with the diverse population in the county. Must have ability to travel locally and out of town for training, as required. Must have valid and insurable driver’s license. DFW, EEOE - $15.00 an hour, Monday – Thursday / 10 hours, schedule may vary.
